Please provide a copy of an identification document that includes your name and address. Our response will be mailed to the address on your identification document. Employees may also receive the report from their HR manager.

How we process your request

The information you provide here will be processed by Kodak staff solely for the purpose of verifying your identity, identifying the information you are requesting, and answering your request.

We will respond to your request within 30 days. If we are unable to complete your request within the 30-day period, we will let you know that and also tell you when your request will be completed.
