Proof of Identity

We will send non-sensitive information associated with the email address given above to that email address. If you would like to access sensitive information or have your non-sensitive information mailed to you, we will need to verify your identity. This process prevents the unauthorized disclosure of your information.

Please provide a copy of an identification document that includes your name and address. The information you provide here will be processed by Kodak staff solely for the purpose of verifying your identity, identifying the information you are requesting, and answering your request.

Our response will be mailed to the address on your identification document. Employees may also receive the report from their HR manager.
