To help us better respond to your requests and give each proposal the full and proper consideration that it deserves, we ask that the following guidelines be observed to ensure a timely response.

How to Submit a Request to the Global Diversity & Community Affairs Office
The Global Diversity Office respectfully requests that all proposals including, but not limited to, sponsorships, advertising, and corporate memberships be submitted no less than six (6) months in advance of the proposed action or event. Please note that due to annual budgets, planning constraints and limited resources, proposals received outside of this timeframe will not be considered. |

Proposals Should Include the Following:
- A brief, but detailed description of the request including how the proposed action or event will enhance the corporate representation and reputation of Eastman Kodak Company with respect to diversity or multicultural audiences.
- The proposed population(s) served: African American, Asian, Disabled, Hispanic, Women, GLBT, Native American, Veterans, etc.
- Marketing Opportunities: Co-sponsor promotional and/or educational activities, Kodak retailer/customer channel promotions, print and television media, etc.
- Fees and Payment Terms/Schedule: All costs Kodak is expected to pay, including sponsorship fee, Value-In-Kind, promotional fees, signage, literature, printing costs, creative/production costs, equipment, merchandising, royalties, licensing, etc.
- Term: Semi-annual, annual, two-year, etc.
- Deadlines: All pertinent deadlines associated with proposed actions or events.
- Attendance: Annual ticket sales/registration, paid and unpaid, trend history for the last three years, and future projections for three years
- Demographics: Age, gender and any other applicable information.
- Contact Information/Phone number/E-mail address.
- A list of other sponsors: Indicate whether potential or committed.
- Any other pertinent information.
Please be certain to include all of the above information so that we can make an informed decision. Based on the limitless volume of requests we receive, the Global Diversity Office will do our best to acknowledge each and every one.
