    Retiree Committee Information

    The U.S. Bankruptcy Court has authorized the Office of the U.S. Trustee to appoint a retiree committee pursuant to section of 1114(d) of the Bankruptcy Code.  Any Kodak retiree, LTD recipient or survivor of such a person who is receiving retiree health and/or survivor benefits is eligible to be considered for the committee.  To be considered, open and review the .pdf file below, complete the Questionnaire Form according to the instructions specified therein, and submit the completed Questionnaire Form as directed by the 12:00 p.m. April 27 deadline.  Any questions concerning the Questionnaire Form should be directed to the Office of the U.S. Trustee.
