Here's how to complete the rebate form to receive the $5 rebate as our thank you for buying our best inkjet paper--just a few easy steps.

| Verify that you are eligible. You are eligible if you purchased one package of Kodak ultima or premium picture paper in the United States between the dates noted on the rebate coupon. | ||

| Fill out the information requested on the rebate coupon. | ||

||

| Cut out the entire UPC/CAT No. from the inkjet paper package. The UPC/CAT No. looks like this: | ||

| Include the original sales receipt, showing the date of your purchase, the item purchased, and the purchase price. | ||
|
|
||

|
|
| Place the completed rebate form, the original UPC/CAT No. from the package, and the original sales receipt in an envelope and mail it to the address shown on the rebate coupon. Allow 6 to 8 weeks for your rebate to arrive. Mail by the date noted on the rebate coupon. | ||

This rebate offer cannot be used in combination with any other Kodak product offer. Cash register receipt and UPC barcode must represent submitter's actual consumer purchase. Rebate is non-transferable. Copies of certificate or proof of purchase or UPC barcode not accepted. Limit one (1) rebate per household, address, group, or organization. Maximum rebate value per household, address, group, or organization is $5.00. Offer void where prohibited by law and is limited to residents of the 50 United States and the District of Columbia (excluding Puerto Rico). Offer excludes resellers of Kodak products and their families. Submitting one original UPC barcode, one original sales receipt, and completed rebate form entitles you to receive a check issued without recourse to the drawer. All obligations of Kodak or its agent will be discharged by issuing the check and mailing it to the address provided by you. Any tax is consumer's responsibility. Cash value 1/20 cent.
